This is the good practice's implementation level. It can be national, regional or local.
About this good practice
Elderlinked assists nurses for a better follow-up of their elderly patients, which represents a more personalised treatment and delays the patientsā departure to a nursing home.
Elderlinked created a digital reminder and warning solution that assists the elderly person daily and communicates directly with their nurse and family.
The system works on the elderly personsā TV, which they are already familiar with and doesnāt require complex interaction from them. The requirements are that the TV should be connected to internet and less than 6-years-old.
The following applications have already been implemented:
ā¢ Medication reminder working along with a smart pillbox
ā¢ Calendar display on a dedicated TV channel showing events, time indicators and weather forecast
ā¢ Other health tips and reminders (hydrate, eat, exercises etc.)
When the system doesnāt receive any answer to a reminder or alert, it automatically notifies the family reference contact or caregiver so they can take actions.
Nurses can also access a secured platform on which they find for each patient, an history of their medication takes, current and past prescription etc.

Resources needed
The monthly cost for the beneficiary would be between ā¬30 and ā¬40 per month, to rent the interactive device connected to their TV.
Evidence of success
The product is prototype is already well advanced, and the two founders hope to be able to commercialize Elderlinked in 2017. The creators of the concept benefited from the financial support of Microsoft and then Digital BW for the start-up phase. They hope to reach around 50 to 100 users in the long term.
Potential for learning or transfer
If the prototype manages to be commercialised, the software can be used anywhere with the same level of technology. Some rural areas might be impaired as this solution needs a good internet connection.
