About this good practice
More than 800K people are hospitalized a year as their chronic disease (eg. Diabetes) is not managed properly. 40% of patients do not use their medications as prescribed and almost 200K people die every year due to improper medicines use. On the other hand, we can see that 98% EU citizens can reach their nearest pharmacy <30mins & to 2/3 of Europeans can reach their nearest pharmacy <5mins. Pharmacies represent the most accessible healthcare providers. This is where our solution is focused: -to support pharmacists in their care for chronic patients; -in conducting their specific health services such as MUR, pharmaceutical care for patients with chronic diseases.
The solution represents a platform where on the one side we provide tools for pharmacist to conduct their health services more efficiently and on the other side we provide tools to patients to support them in their treatment. Pharmaceutical service takes place in the pharmacy. In addition, the patient is also monitored and supported remotely. Both,patient and pharmacist can also contact physician via the platform.
Our solution? Active inclusion of patient in the treatment process. Technology goes on, however usually patients are still excluded from the healthcare process which is key to treatment success.
The solution was granted the Seal of Excellence in 2016 from EC within Horizon 2020.The solution is currently being used by pharmacists and patients covered by cca 100pharmacies and we are spreading the network

Resources needed
We are in phase where we would like to introduce this solution to foreign markets as well. This related to substantial financial burden.
Evidence of success
It’s successful as it implements both, clinical aspects -pharmaceutical care-PC&MHM by addressing several challenges - social and biomedical/clinical aspects. Combination is crucial.

Potential for learning or transfer
Current solutions usually fail in cases when only partial solution is offered (only pharmaceutical care service without tools for easiest implementation, or only Mobile Health Management tools without medication services demanding included functionalities) but not solve the whole complexity of factors influencing introduction of new medication approach.
Patients are not sufficiently involved in the healthcare process.
Healthlord Pharmacy project addresses several challenges i.e. social (new ways of communication, inclusion of patients in the healthcare process, patient awarding systems) and biomedical aspects (pharmaceutical care has proven/published beneficial effects).
Healthlord Pharmacy is ready and can be easily transferred to other regions since work of pharmacies, their role and standards are comparable throughout Europe.
