The aim of the project is to improve health care processes and the health-related opportunities of the citizens.
Due to low socioeconomic status, people living in Hamburg’s neighbourhoods of Billstedt and Horn have lower health-related chances. In these parts of the city, there are more social welfare recipients, more people with a migrant background etc. These citizens get ill earlier in life and have a lower life expectancy. Likewise, access to healthcare is limited.

The chance to live a healthier life in Billstedt and Horn will be improved. In order to achieve that, the project partners will implement a patient oriented and interdisciplinary health network. Aims:
o Empower patients to take more responsibility for their health
o Decrease the workload of general practitioners
o Use resources efficiently
o Increase quality of care

The project partners carry out various interventions aimed at strengthening the collaboration between the medical and social sectors. Main interventions:
o A health kiosk based at a central location
o Innovative IT-communication-solutions
o Integrated healthcare management
o Training of physicians in drug safety

A key element is the so-called “Gesundheitskiosk”. It is a place where person find help, if they need health related support. For example a network of translators exist, that can translate diagnoses. Furthermore, guidance about the health-system and related offers can be given. This is very important, because Germany has a decentralized health systems.

Healthcare providers and health insurances are the main stakeholders.

Evidence of success

The health kiosk has shown how successful cross-sectoral cooperation can be. An analysis of the project results was the basis of Hamburg’s parliament decision, to build up more infrastructure in districts with a need for support. Data like how many person looked for help, communication with health professionals, and so on were taken into account. The company Optimedis is analysing public health data as well.

Potential for learning or transfer

The project gives an example, how person that are not fully integrated not speaking mother tongue, underprivileged educational backgrounds, or other reasons, can get help in health and health related sectors. This knowledge has an important factor on your health status and healthy aging.

Has this good practice been adopted in other regions around the country or beyond?
Based on the experience of the two projects Gesundes Kinzigtal and INVEST Billstedt/Horn, Optimedis developed an integrated care model, that was selected as one of four good-practice-models from the EC. The good-practice-models will be analysed and implemented in different regions in the EU during the Joint Action JADECARE.
